Proof of citizenship required - Original birth certificate or original certificate of naturalization (no copies)
Original birth certificates required for children (no copies)
Both parents must be present for children under 16 (One parent, if only that parent is in the birth certificate)
Please bring Passport Photo
Secondary ID (ie. Driver's license)
